Our goal is to help you make a positive, financially sound long term plan of care that will meet the senior's individual needs. And we strive to help maintain the senior's independence for as long as possible in his or her home of choice.

Inter-disciplinary Approach
As you 'shop around" for Geriatric Care Management services (and we believe you should) you'll probably notice that many of the professionals are either nurses or "certified care managers" (who may have backgrounds in any of several fields and perhaps took a series of courses to become certified). We at ElderChoice are different. Founded by a Masters-educated, Licensed Clinical Social Worker with over 20 years of experience in providing mental health counseling and helping clients and families, ElderChoice offers care management and professional counseling by a Licensed Clinical Social Worker. We also offer consultations with a Registered Nurse as needed.

Support and Funding
Finally, because our founder is an LISW-CP Clinician, we are able to provide therapy and mental health counseling to our patients and families, and some of your services may be covered by Medicare,
Medicaid or private insurance. We accept Medicare assignment.
